Study: Women Wait 14 Years for a Bleeding Disorder Diagnosis. Men Wait Two.

Women with inherited bleeding disorders wait an average of 14 to 16 years to receive a diagnosis. Men with the same conditions wait about two. That gap is the central finding of a new Lancet Haematology Commission on global female blood health, presented in Paris in July 2026 by a team led by a Cleveland Clinic hematologist.

The Commission points to a structural failure at the first point of contact. Heavy menstrual bleeding affects up to one in three women and is frequently the earliest sign of an inherited bleeding disorder. Instead of triggering a diagnostic workup, it is routinely categorized as a normal variation. The bleeding gets treated. The disorder behind it does not.

The 14-year gap is not explained by biology. It is explained by what happens when women describe their symptoms and what clinicians do next. And the Commission is proposing something specific to fix it.

The Commission’s proposal centers on universal screening for heavy menstrual bleeding and iron deficiency at primary care visits. The argument is that these two markers are cheap to test, widely available, and predictive enough to catch most inherited disorders before they go undetected for another decade. The group has set a target of reducing average diagnosis time for women to under two years by 2035, matching the current standard for men.

For patients, the practical takeaway is this: heavy periods are not automatically normal. A period that requires changing protection every hour, that includes large clots, or that causes significant fatigue may warrant a conversation about bleeding disorder screening, not just hormonal management. Asking your doctor about a von Willebrand factor test or a full coagulation panel is a reasonable first step, and one the Commission is pushing providers to make routine.
