Study: In a Pregnancy Trial, Strength Training Eliminated Gestational Diabetes

In a randomized controlled trial of just over 200 pregnant women who were overweight or obese, not one woman who completed a supervised strength-training program developed gestational diabetes. In the control group, 7.3% did.

The difference extended to their babies: no infants in the strength-training group were born oversized, compared to 6.3% of controls. Women in the exercise group also reported better quality of life. The trial, conducted in Spain, ran the program throughout pregnancy with supervised sessions.

The trial is small, and results hinge on adherence — women who dropped out weren’t counted in the strength-training group. That’s a meaningful caveat, because sticking with a supervised program throughout pregnancy requires access and support that most women don’t have. But a zero gestational diabetes rate in the compliant group is a signal worth taking seriously, even if replication in larger, more diverse populations is needed before clinical guidance shifts.

The mechanism researchers point to: resistance training improves insulin sensitivity and glucose uptake in muscle tissue, effects that are particularly relevant during pregnancy, when the body’s demand for insulin regulation increases significantly. If the signal holds, this could reshape how high-risk pregnancies approach exercise prescriptions — and how obstetricians talk to patients about strength training as a clinical intervention rather than optional wellness.
