Menopause Doesn’t Exist in Most Electronic Health Records

When a clinician opens your chart, they can usually see your blood pressure trend over five years, your medication history, your vaccination record. What they almost certainly cannot see: whether you’re in menopause, when it started, or what symptoms you’ve reported.

A new analysis found that menopause status is rarely captured as structured data in electronic health records. Instead, it gets inferred from age, a billing code, or a prescription for hormone therapy. None of those proxies tell a clinician what’s actually happening to you, when it started, or how it’s progressing.

The result is that most menopause care is happening without a reliable record underneath it — and most menopause research is being built on the same unstable foundation. Researchers are calling for a standardized way to document menopause status and symptoms before the data gets any worse.

The result is that most menopause care is happening without a reliable record underneath it — and most menopause research is being built on the same unstable foundation. Researchers are calling for a standardized way to document menopause status and symptoms before the data gets any worse.

The practical consequence: if your symptoms change between visits, there’s no longitudinal record for a clinician to compare against. If you switch providers, the new one starts from scratch. If you’re part of a study, researchers are working with data that was never designed to capture what they’re trying to measure.

This is a systems problem, not an individual one. The fix isn’t to ask patients to track their own symptoms more carefully. It’s for electronic health record platforms to build menopause into the structured data layer the same way they track blood pressure, cholesterol, or BMI — as a condition with a timeline, not an afterthought inferred from age. Until that happens, menopause care will keep flying without instruments.
